New Clients

For new client referrals or to restart meals for a consumer please refer to the information below to have a timely response on client start-up. Please make sure all client information is provided accurately:

Area Agency on Aging

  • If you are a Comcare or Passport case manager, all you need to provide is consumer’s demographics and amount of meals being requested.
  • If you are a Buckeye or Molina case manager, we need a referral with demographics and amount of meals being requested. An authorization/service plan from the specific funding will also need to be sent to us before we can set a start date.

Catholic Social Services

  • All you need to provide is demographics and amount of meals being requested

Molina (direct)

  • All you need to provide is demographics, amount of meals and authorization

Buckeye (direct)

  • All you need to provide is demographics, amount of meals and authorization and service plan

Ohio Medicaid Waiver

  • You will need to provide a referral with client demographics, requested meal amount, confirmation of physician, ICD 10 codes, and service plan with confirmed information being requested.
Existing Client Updates

Make sure that you send us a written copy of the update for your consumer either by email or fax. Email is best as this avoids any missed communication.

Authorized Signers

If your client is unable to sign:

  • Area Agency on Aging and Catholic Social Services need to have supervisor confirmation on other signers. Please note that you are authorizing someone else to sign which guarantees payment if a client and family do not communicate information to you that would have their funds used elsewhere.
  • Buckeye / Molina (direct) and Ohio Medicaid Waiver case managers can request this information. Please note that you are authorizing someone else to sign which guarantees payment if a client and family do not communicate information to you that would have their funds used elsewhere.
